Abstract

췌장암의 가장 효과적인 치료는 수술적 절제이나 10-20%에서만 가능하다. 그 이유는 췌장은 복막 후강에 위치하고 있어 암이 발생하더라도 초기에 특이적인 증상이 없고, 암 조직이 빠르게 성장하여 진단 당시 이미 진행된 경우가 많기 때문이다. 절제 후에도 재발이 흔하여 근치적 절제수술 후에도 5년 생존율이 20% 이하이다. 절제 이외의 치료 방법은 효과가 적어 전체 췌장선암종 환자의 5년 생존율은 5% 이하로 매우 예후가 나쁜 종양이다.

정확하고 엄격한 수술 전 병기 판정과 절제가능성 평가에 따른 병기별 치료는 필수적이다. 절제가 불가능하나 원격전이가 없는 국소진행성 췌장암은 30-40%에서 관찰되며 중앙생존기간은 치료를 안 한 경우 6-8개월, 치료한 경우 10-12개월 정도이다. 이들 국소진행성 췌장암은 생존율 향상의 여지가 많아서, 보다 나은 치료 효과를 얻기 위해 가장 적극적이고 집중적인 관심이 필요한 상태의 췌장암이라 할 수 있다. 췌장암의 절제술에 있어서 경계성 절제가능성 췌장암의 개념의 등장의 배경은 낮은 R0 절제율과 힘든 절제 후의 나쁜 예후에 있다. 최근 다중검출 CT의 발전으로 새로이 분류된 경계성 절제가능성 췌장암군은 절제연 양성의 위험성이 높아 다학적 치료법의 좋은 적용군이라 할 수 있다. 최근 R1이나 R2 절제의 가능성이 높은 경우 무리한 수술보다는 수술 전 항암요법과 방사선요법 후 영상학적으로 진행하지 않거나 개선되어 혈관침습이 국소적인 경우 혈관 절제나 재건을 포함한 근치적 수술을 시도하여 비교적 좋은 결과들이 보고되고 있다.

수술 전 화학요법 또는 화학방사선치료는 수술 후 치료에 비해 미세원격전이가 있어 나중에 진행하여 명백한 원격전이가 보이는 환자들에게는 불필요한 절제수술을 막을 수 있다. 또한 수술 후 보조요법을 못하는 환자가 1/3에 달하는 상황에서 전신상태가 비교적 좋을 때 여러 다양한 치료를 시도할 수 있다. 최근 췌장암에 효과적인 몇몇 약제들이 소개되면서 이들 약제의 방사선치료와의 병합치료에 대한 기대 역시 증가하고 있다. 하지만 아직 그 정의에서 각 병원마다 차이가 많고 치료 및 효과의 판정에서도 확립되지 않은 부분이 많아 잘 짜여진 기준을 가진 전향적 무작위 대조 연구가 필요하다.

또한 수술 전 항암 또는 방사선치료 효과를 정확히 평가할 수 있는 영상 검사나 병리학적 기준이 필요하며 향후 개개인에 맞는 맞춤형 치료의 개발 및 효과를 볼 수 있는 환자군을 선택하는 것이 필요하겠다.

Trans Abstract

Surgical resection offers the only chance of cure for nonmetastatic exocrine pancreatic cancer. However, only 15 to 20 percent of patients have potentially resectable disease at diagnosis; approximately 40 percent have distant metastases, and another 30 to 40 percent have locally advanced unresectable tumors. Typically, patients with locally advanced unresectable pancreatic cancer have tumor invasion into adjacent critical structures, particularly the celiac and superior mesenteric arteries. The optimal management of these patients is controversial, and there is no internationally embraced standard approach. Therapeutic options include chemoradiotherapy or chemotherapy alone. While it is reasonable to restage and reevaluate the potential for resectability after neoadjuvant therapy, the frequency of a complete resection and long-term survival is low for patients who initially have categorically unresectable tumors. Others have disease that is categorized as “borderline resectable.” While these patients are potentially resectable, the high likelihood of an incomplete resection has prompted interest in strategies to “downstage” the tumor or to increase the likelihood of a margin-negative resection prior to surgical exploration using neoadjuvant therapy. The rationale for neoadjuvant therapy is as follows. First, it is to improve the selection of patients for whom resection will not offer a survival benefit (i.e., those who rapidly progress to metastatic disease during preoperative therapy). Second, it is to increase rates of margin-negative resections, which is the major goal of surgery. Third, it is to start an early treatment of micrometastatic disease. Initial attempt at downstaging with chemotherapy, chemoradiotherapy, or a combination followed by restaging and surgical exploration in responders rather than upfront surgery is suggested.

서 론

2016년에 발표된 한국 중암암등록본부 자료에 의하면 2013년에 우리나라에서 췌장암은 연평균 5,511건 발생하여 전체 암발생의 2.4%로 8위를 차지하였고, 인구 10만 명당 조발생률은 10.9건이었다. 최근 10년간 발생률 및 사망률은 지속적으로 증가하고 있다. 현재까지 췌장암의 가장 효과적인 치료는 수술적 절제이나 10-20%에서만 가능하다. 그 이유는 췌장은 복막 후강에 위치하고 있어 암이 발생하더라도 초기에 특이적인 증상이 없고, 암조직이 빠르게 성장하여 진단 당시 이미 진행된 경우가 많기 때문이다. 절제 후에도 재발이 흔하여 근치적 절제수술 후에도 5년 생존율이 20% 이하이다. 절제 이외의 치료 방법은 효과가 적어 전체 췌장선암종 환자의 5년 생존율은 5% 이하로 매우 예후가 나쁜 종양이다[1].

절제가 불가능한 암은 원격전이는 없으나 주변 조직(특히 복강동맥[celiac artery, CA]이나 상장간막동맥(superior mesenteric artery])으로의 침윤에 의해 절제가 불가능한 국소진행형과 이미 타 장기로 원격전이가 동반된 경우로 구분한다. 원격전이가 동반된 췌장암의 경우는 40-60%를 차지하며 중앙생존기간은 5-7개월 정도이다. 절제가 불가능하나 원격전이가 없는 국소진행성 췌장암은 30-40%에서 관찰되며 중앙생존기간은 치료를 안 한 경우 6-8개월, 치료한 경우 10-12개월 정도이다. 이들 국소진행성 췌장암은 생존율 향상의 여지가 많아서, 보다 나은 치료 효과를 얻기 위해 가장 적극적이고 집중적인 관심이 필요한 상태의 췌장암이라 할 수 있다. 최근 CT의 발전으로 암의 국소 침범 정도를 더욱 세분화하여 경계성 절제가능성을 지닌 췌장암(borderline 혹은 marginal resectable pancreatic cancer)을 구분하여 근치적 절제가 가능할 수도 있다고 제시되었다[2-6]. 이들에서 주변 혈관 또는 장기를 합병 절제하는 확대 절제방법과 선행 항암방사선 치료 등으로 예후를 향상시키려는 시도가 보고되고 있다.

본 론

1. 배경

일부 국소 진행된 종양에서 절제 후 절제연(resection margin)에 종양세포 음성의 소견이 발견되어 좋은 예후를 보이는 보고들도 있는 반면에 수술 전에는 절제가 가능할 것으로 여겨졌으나 실제 절제연 음성의 소견을 얻기가 어려운(R1 또는 R2 resection) 경우들도 있다. 절제연의 암세포 존재 여부는 림프절전이와 함께 중요한 예후인자로서 R1, R2절제의 경우(수술적 절제를 시도하였으나 근치적 절제[R0절제]를 못한 경우) 근치적 절제가 된 군보다 예후가 나쁘고 국소진행형 환자와 예후가 비슷하다(Table 1)[7-9]. 따라서 수술 전 근치 절제가능성 여부의 판정이 매우 중요하다. 해부학적 구조에 따른 근치적 절제의 어려움과 최근의 다중 검출 CT (multi-detector CT)의 발전은 절제 가능한 종양과 절제 불가능한 종양 이외에 양쪽에 정확히 맞지는 않은 이 두 군의 중간 정도에 속하는 군을 경계성 절제 가능한 종양으로 분류하게 되었다.

수술적으로는 절제가 가능하나 실제 절제연 음성의 소견을 얻기가 어려운(R1 또는 R2 절제) 환자에서 단순한 수술적 치료의 한계를 인정하고 수술 이전에 다양한 치료 방법을 이용하여 근치적 절제율(R0 절제)을 높이고자 시도되고 있다.

2. 수술 전 병기결정-절제가능성 평가

CT, MRI, 내시경초음파검사, PET 검사, 혈청 종양표지자 검사 등의 다양한 검사를 시행하여 수술 전 정확한 병기를 선정하기 위한 최선의 노력이 필요하다. 이 중 절제가능성 평가에는 다중검출 CT가 가장 정확하다[10]. ERCP 후 췌장염이 발생하면 절제가능성 판단이 어려워지므로 반드시 담즙배액 시술 전에 CT를 시행하여야 한다. 수술 전 CT 검사를 주의 깊게 판정하여 특히 R2 절제가 되지 않도록(수술하지 말고 피하도록) 해야 한다. CT의 가장 큰 제한점은 복막 또는 간의 작은 전이는 예민도가 낮다는 것으로 약 20-30%에서 수술 전 절제가능형으로 판단하였으나, 개복수술이나 복강경에서 복막 또는 간에 매우 작은 전이성 결절이 관찰된다.

진단적 복강경 후 전이가 없음을 확인 후 개복하였으나 절제를 못한 경우는 주요 혈관침범이 원인인 경우가 가장 많다. 수술 전 CT 검사에서 혈관 침범 정도에 따라 절제가능성을 판단한 여러 보고가 있다[11-13]. 누구나가 주요 혈관 침윤이 분명하다고 인정할 수 있는 객관적인 방사선 소견을 보이는 경우에서는 별다른 이견이 없어, 동맥 주변 중요 혈관을 완전히 감싸는 침윤(encasement)은 종양의 근치적 절제가 불가능하다고 판단한다. 일반적으로 180도 이상의 상장간막동맥침윤, 대동맥침윤, 재건할 수 없을 정도의 상장간막정맥(superior mesenteric vein), 문맥(portal vein)의 폐색은 절제가 불가능하다. 반면에 혈관의 일부와 인접(abutment)해 있을 경우 실제 절제가 가능한 병변인지 여부를 정확히 판단하기 어려울 수 있다. 이 경우 절제수술 후 절제연(특히 후복막[retroperitoneal]) 상장간막동맥 주위)이나 림프절에 종양세포가 남을 위험성이 더 높으며 이 경우 R0 절제에 비해 예후가 나쁘다. 종양의 주요 동/정맥 접촉 각도가 180도 미만이면서 혈관 내로의 직접 종양 침윤은 없는 것으로 여겨지는 경우 경계성 절제가능성이 있는 것으로 판단한다. 경계성 절제가능 췌장암은 기술적으로 종양의 절제가 가능할 것으로 판단되나 현미경적 절제연 양성(R1 절제)이 될 확률이 높아 종양학적 측면에서 근치적 절제 가능성이 낮은 경우를 뜻한다. 아직까지 경계성 절제가능 췌장암에 대한 정확한 정의도 차이가 있고 치료의 결과에 대해서도 논란의 여지가 있어 앞으로 더 많은 연구가 필요하며 여러 연구 결과마다 절제기준의 차이가 있어 결과해석에 주의를 요한다.

3. 경계성 절제가능성 췌장암의 정의

아직까지 경계성 절제가능성을 가진 췌장암의 기준에 대한 명확한 정의는 확립되지 않았다. 경계성 절제가능성 췌장암의 정의에 대해서는 National Comprehensive Cancer Network 기준과[10] 활발히 연구하고 있는 MD Anderson Cancer Center 또는 최근 미국에서 진행 중인 전향적인 다기관 연구 등의 기준에 약간의 차이가 있다(Table 2)[3,10,14-17].

AJCC 7판 병기 분류에서는 절제 가능군(1, 2 병기)과 절제 불가능군(3, 4병기)으로 나뉘어져 있다. 췌장주변 장기인 위, 비장, 대장, 주변 혈관 등의 침윤이 5판에서는 T4이었던 것이 6판부터는 복강동맥, 상장간막동맥의 침윤으로 T4가 제한되었다. 이러한 변화는 종양의 주변장기 침윤 자체보다는 근치적 절제가 가능한지 여부가 가장 중요한 예후인자로 여겨지기 때문이다. 경계성 절제가능성 췌장암의 경우 stage II의 일부군 중 간문맥-비정맥 합류부(confluence)를 침범한 군, 총간동맥 또는 고유간동맥을 침범한 군 중 일부, 복강동맥 및 상장간막동맥의 침범 여부가 불투명하고 접근되어 있는 경우 등이 포함될 수 있으며 stage III 중 복강동맥 및 상장간막동맥의 침범이 일부 있는 경우가 포함될 수 있다.

주요 혈관 침습 여부를 기준으로 하는 상기 기준들은 주관적인 요소가 많고, 연구자마다 이를 적용, 해석하는 기준이 상이하여 경계성 절제가능성을 가진 췌장암에 대한 지금까지의 연구 결과를 일관되게 비교 분석하는 것이 어려운 실정이다. 따라서, 향후 진행될 임상 시험 결과의 일관된 해석을 위해 경계성 절제가능성 췌장암에 대한 객관적인 정의 확립이 중요하다.

4. 수술 전 항암요법 또는 화학방사선요법

췌장암에서의 수술 후 보조요법이라는 표준적 치료에도 불구하고 그 생존 성적은 매우 비관적이며, R0 절제가 되지 않는 무리한 수술은 좋지 않은 예후와 관련됨이 보고되고 있다. 다른 고형 암에서처럼 국소적인 췌장암에서도 수술 전 화학요법 또는 화학방사선요법의 가능성을 통해 절제 후 성적을 좋게 하려는 시도가 이루어 지고 있다.

수술 전 화학요법 또는 화학방사선 치료는 수술 후 치료에 비해 이론적으로 수술에 따른 혈관손상이 없는 상태에서 방사선치료가 진행되어 산소공급이 원활하므로, 암세포의 방사선감수성(radiosensitivity)이 더 높을 것으로 기대된다. 또한 미세 원격전이가 있어 나중에 진행하여 명백한 원격전이가 보이는 환자들에게는 불필요한 절제수술을 막을 수 있다. 또한 수술 후 보조요법을 못하는 환자가 1/3에 달하므로 전신상태가 비교적 좋을 때 여러 다양한 치료를 시도할 수 있다. 특히 R0 절제를 하기 어려운 경계 절제성 췌장암에서는 혈관-종양의 경계면의 암세포를 박멸하여 R1 또는 R2 절제율을 감소시키고 R0 절제율을 높이고자 함이다. 최근 췌장암에 효과적인 몇몇 약제들이 소개되면서 이들 약제의 방사선 치료와의 병합치료에 대한 기대 역시 증가하고 있다[18,19]. 하지만 식도암이나 직장암의 경우는 선행치료의 효과가 수술 효과에 근접하는 수준을 보여 표준 치료로 인정되고 있는 것과는 달리 아직까지 췌장암에서는 낮은 반응률로 인하여 논란이 있다. 또한 국소질환 진행 또는 독성으로 인한 절제 연기나 불가능의 우려가 있다. 췌장암에서의 수술 전 화학방사선요법을 위해서는 수술 전 정확한 병기평가와 병리학적 진단(주로 초음파내시경 세침 흡인술이나 초음파내시경 유도하 세침 흡인술) 및 담즙 배액술(선행치료 시에는 플라스틱 배액관보다는 주로 금속 배액관)이 필수적이다.

아직까지 표준 치료법은 제시되지 못하고 있다. 5-FU, gemcitabine이나 capecitabine이 주로 사용되고 있으나 우리나라에서는 방사선민감제로서는 비급여인 실정이다. 현재로는 방사선 총량은 관련 항암제에 따라 좌우되는 실정이며 아직 국소조절이나 생존기간과 관련된 용량-효과 상관관계, 방사선 총량 등에 대한 논란이 있다. 연구마다 방사선량, 방사선치료 범위, 항암화학제제의 용량 및 방사선치료와의 병용 시기 등에 차이가 있고, 이로 인해 치료 독성 및 성적도 일치된 결과를 보여주지는 못하고 있어, 향후 추가 연구가 필요하며 아직까지는 이에 대한 명확한 결론은 내려지지는 않은 상태이다. 방사선의 조사량, 방사선과 약물 치료의 시점, 약제의 종류 및 투여방법 등 많은 문제점들이 해결되어야 한다. 아직 무작위 대조 연구가 부족하고, 대상 환자의 수가 제한적이며, 결과가 일관되지 않고, 추적관찰기간이 비교적 짧으며, 정확한 병기 판단을 위한 검사법에 차이가 있는 등 여러 문제점이 있다.

방사선과 항암약물치료의 시점에 대해서도 논란이 있다. 국소질환으로 생각되었던 췌장암 환자의 약 20-30%에서 항암제치료 후 전이가 발견되므로 최근 연구에서는 먼저 항암화학요법 수개월 시행(유도항암요법) 후에 화학방사선 병행요법을 시도하고 있다. 미세원격전이가 있어 나중에 명백한 원격전이로 진행하는 환자들은 방사선치료를 안하고 항암치료를 지속하며, 질병이 진행하지 않고 방사선치료에 이득을 볼 수 있는 환자만 선택하고자 함이다[20-24]. 이들 연구들은 대개 2상 임상연구이거나 후향적 연구들로 아직은 결론을 내리기 어렵다. 최근 무작위 대조 연구에서는 gemcitabine을 이용한 유도항암요법 연구에서 화학방사선 병행요법만 환자군과 생존기간의 차이가 없었다[25].

췌장암에서의 수술 전 화학방사선요법의 반응률은 3.4-44%, 절제율은 8.3-64.2%로 다양하게 보고되나 실제로 분명한 국소진행형 환자에서는 병기감소(down-staging) 또는 크기감소(down-sizing)로 절제불가능에서 절제가 가능해지는 경우는 매우 드물다[26-29]. 각 연구들에서 환자선택기준, 영상의학적 병기판정, 수술 전 조직획득, 수술 전 담도 및 십이지장폐쇄의 치료, 절제기준, 수술기법, 병리학적 평가, 항암약제 그리고 방사선기법 등이 다양하여 결과의 해석에 주의를 요한다. 화학방사선요법에 관한 연구는 대개 절제가능성이 있는 환자와 경계성 절제가능성을 지닌 환자, 그리고 국소진행형 환자가 혼합되어 있는 경우가 많다. 선행보조요법 또는 신보강요법(neoadjuvant)이라는 용어는 영상검사에서 절제가 가능한 환자에 국한되어 사용하여야 결과의 혼동을 피할 수 있겠다.

최근 경계성 절제가능성 췌장암에서도 수술 전 화학방사선요법 이후 수술하는 것이 점차 보편화되고 있는 추세이다. 특히 이들은 절제가능군보다 미세전이로 인한 수술 후 조기 전이 가능성이 더 높고 혈관 등의 절제수술로 수술 후 합병증 가능성도 더 높다. 이들에서 수술 전 화학방사선요법을 시행 후 15-50%에서 절제수술을 시행하여 중앙생존기간이 20-40개월이라고 보고하고 있다(Table 3)[14,30-41]. 대개의 연구가 2상 연구이거나 후향적 연구들로 아직은 결론을 내리기 어려우나 최근에는 많은 임상의들에게 보편적으로 받아들여지고 있다[42,43]. 최근 진행성 환자에서 반응률이 좋은 FOLFIRINOX 제제나 Gemcitabine+nab-paclitaxel를 이용하여[44,45] 선행요법을 시도하는 연구도 활발하다[46-48].

항암치료나 방사선치료와의 병용 여부나 치료순서, 시기 등에 따라서도 차이나 장단점이 있다(Table 4)[4]. 향후 절제수술 후 보조요법과의 무작위 대조 연구가 필요한 실정이다.

종양의 진행범위 이외에도 수행능력, 체중감소, 식욕부진, 혈청 CA19-9 등 다양한 요소에 의해 예후 차이가 날 수 있다. 최근에는 해부학적인 경계성 절제가능성 췌장암 환자 이외에 전이 여부가 불분명하거나 담관염, 황달 등의 내과적 동반질환으로 인해 영양이나 전신상태가 좋지 않은 환자들도 같이 포함하여 수술 전 화학방사선요법으로 선행치료 후의 CA19-9와 영상소견으로 판단하여 전이가 발생하지 않고 전신상태가 호전된 경우 수술하여 비교적 좋은 예후를 보고한 바 있다[15]. 수술 전 화학방사선요법으로 이익을 받을 수 있는 환자를 보다 적극적으로 선택하는 시도로 결과해석에 주의를 요한다.

5. 선행치료 후의 반응평가

선행치료 후 미만성 췌장의 염증, 섬유화, 부종 등의 2차성 변화로 인해 영상학적 검사로는 종양의 범위를 과장되게 평가하는 경향이 있어 절제가능성을 정확히 예측하기가 어렵다[49,50]. 실제로 영상학적으로 병기나 크기가 감소하는 경우는 드물지만 절제수술을 시행하면 R0 절제를 얻는 경우가 많다[51,52]. 따라서 수행상태가 좋은 환자에서 원격전이 같은 질환의 악화가 없으면서 CA19-9 수치가 감소하는 경우 등에서는 적극적으로 절제수술을 시행하는 것을 권장한다. 추후 PET-CT나 다른 기능성 영상 검사로 절제가능성을 예측하는데 도움을 받을 수 있을 것으로 여겨진다.

6. 췌십이지장절제술과 절제연

일반적으로 췌장은 해부학적으로 문맥, 상장간막정맥, 상장간막동맥, 복강동맥, 또는 간동맥(hepatic artery) 등 매우 중요한 혈관들의 중심에 위치해 있고 많은 림프절 및 신경절이 교차하는 특성을 지니고 있어 수술시 기술적인 어려움으로 근치적 절제라는 종양학적 원칙에 충실하기가 어렵다[53].

그러나 이 중 간문맥이나 상장간막정맥의 합병 절제는 상장간막동맥, 복강동맥, 또는 간동맥의 합병 절제와 다른 양상을 보인다. 문맥이나 상장간막정맥을 침범하는 경우에는 각 병원 외과의사에 따라 차이가 있으며 혈관 절제 및 인조혈관(graft)수술을 하기도 한다. 병변의 침범이 국소적으로 재건술이 가능한 경우에는 근치적 절제를 방해하지는 않는다. 이들 정맥의 절제는 상기 동맥의 절제보다 기술적으로 용이하며 술 후 합병증의 발생률도 적은 편이고 덜 치명적이다[54]. 종양이 혈관의 내강까지 침윤했는지 여부와 정맥침범 정도에 따라 실제 절제 후에 예후에 차이가 있다[31,55-57]. 많은 보고에서 상장간막동맥이나 복강동맥의 침범 없이 간문맥이나 상장간막정맥을 국소적으로 침범한 예에서 좋은 성적을 보고하였고 실제 AJCC 7판 병기분류에서도 이러한 결과가 반영되어 국소적 간문맥이나 상장간막정맥 침범의 절제를 표준적 수술로 포함시켰다. 수술 후 이환율이나 사망률이 적으면서 R0 절제를 얻고 절제된 정맥의 혈관벽에 직접 침범이 없다면 이상적이다. 하지만 전술한 바대로 직접적인 혈관 침범 여부는 선행치료 후의 영상 검사로 정확히 평가하기 어려운 경우가 많다.

대개의 경우 상장간막정맥의 침범 없이 상장간막동맥을 침범하는 경우는 드물다. 이론적으로 상장간막동맥을 국소적으로 침범하는 경우 절제 및 재건하는 수술은 가능하다. 하지만 전술한 바와 같이 상장간막동맥이나 복강동맥의 위치는 많은 림프선 및 신경절이 둘러싸고 있어 실제 근치적 절제가 간문맥이나 상장간막정맥보다 어렵고 실제 육안적으로 근치적 절제가 되었다 하더라도 현미경적 종양 침범이 있는 경우가 많다. 또한 상장간막동맥 절제를 포함한 광범위 절제시 신경 손상 등으로 수반되는 조절되지 않는 설사나 단장(short bowel) 증후군 등의 합병증은 환자의 삶의 질을 심하게 저하시킬 수 있으며 추후의 항암치료에 어려움이 따른다. 일반적으로 대동맥 근처의 신경총과 떨어져 있는 총간동맥(common hepatic artery)이나 고유간동맥(proper hepatic artery)의 국소적 침범은 기술적으로 재건이 가능한 경우 절제의 금기는 아니다. 그러나 위십이지장동맥(gastroduodenal artery)을 침범하고 고유간동맥도 침범한 경우는 근치적 절제가 어렵다. 환자에 따라 해부학적 변이가 있어서 절제하더라도 간혈류에 큰 문제가 없거나, 연결이 가능한 경우 등에서는 R0 절제가 가능하다면 동맥 합병 절제를 고려할 수 있다. 해부학적 변이로 우간동맥이 상장간동맥에서 기시하는 경우 종양에 의해 침범되는 경우도 있어 절제 후 여러 가지 방법을 이용해 재건이 필요하다.

복강동맥침범 역시 상장간막동맥 침범과 같은 양상으로 취급되어 종양이 감싸고 있을 때 근치적 절제는 어렵다고 여겨진다. 또한 절제시 간괴사, 위장 허혈 등의 심각한 합병증을 발생할 수 있다. 그러나 때로 복강동맥의 총간동맥 부분으로만 침범했을 경우 복강동맥을 절제해 내는 적극적 수술을 시도하기도 한다.

췌장암의 근치적 절제가능성은 외과의의 경험과 철학에 따라 차이가 있다. 아직까지 동맥 절제를 같이 시행하는 것이 생존율 향상을 가져오는가에 대한 확실한 증거는 부족한 실정이다.

7. 병리학적 평가

췌십이지장절제술 도중에는 촉진만으로는 종양과 상장간막정맥-문맥 합류부, 상장간막동맥의 외측이나 뒤쪽과의 관계를 미리 알 수 없으며, 췌장과 십이지장을 절제 후에야 절제연의 상태를 파악할 수 있다. 하지만 일단 췌장과 십이지장을 절제하면 R0 절제가 안되더라도 췌십이지장절제술을 끝까지 진행할 수밖에 없다. 절제연 특히 주로 상장간막동맥 우편, 기시부에서 3-4 cm에 걸친 구상돌기(uncinate process)와의 사이의 신경 및 연부조직 절제연이 후복막 절제연으로 외과의사가 잉크로 칠해서 병리과에 보내게 된다. 실제 육안적으로 근치적 절제가 되었다 하더라도 이 부위에 현미경적 종양 침범이 있는 R1 절제가 발생한다. 상장간막동맥은 주위에 상장간막동맥의 양측으로 감싸는 자율신경과 림프선 등이 있고 연하여 주요 신경절(ganglion)과 연관되어 있어 상장간막동맥을 전체적으로 감싸는 종양의 경우 대부분 이러한 신경 및 림프선에 전이가 되어 있어 주변 암세포까지 근치적 절제가 되기는 매우 어렵다. R1이나 R2 절제 여부는 림프절전이와 함께 매우 중요한 예후 인자이나 여러 연구들에서 절제연의 상태에 대해 기준이 다양하고 모호하여 결과가 16-85%로 매우 다양하다. R1 절제의 일부에서는 실제로 R2 절제였을 가능성이 있다. 병리의사로서는 R1 절제와 R2 절제를 구분할 수 없어 외과의가 수술기록지에 R2 절제 여부를 판정하여야 한다.

선행치료 후 췌장의 염증, 섬유화, 부종 등의 2차성 변화로 인해 병리학적으로도 다른 암종에 비해 선행치료의 평가가 어려운 경우가 많다. 종양괴사와 섬유화가 중첩되어 있고 종괴 중에 암세포는 군데군데 산재하며 종괴 대부분은 섬유세포로 구성되어 있다. 또한 종양감소를 평가하는 체계도 다양하다[58]. 실제로 병기나 크기가 감소하는 경우는 드물다. 최근 병리학적으로 치료반응이 좋은 경우 예후가 좋다고 보고하고 있다[59,60].

결 론

정확하고 엄격한 수술 전 병기 판정과 절제가능성 평가에 따른 병기별 치료는 필수적이다. 췌장암의 절제술에 있어서 경계성 절제가능성 췌장암의 개념의 등장의 배경은 낮은 R0 절제율과 힘든 절제 후의 나쁜 예후에 있다. 최근 다중검출 CT의 발전으로 새로이 분류된 경계성 절제가능성 췌장암군은 절제연 양성의 위험성이 높아 다학적 치료법의 좋은 적용군이라 할 수 있다. 최근 R1이나 R2 절제의 가능성이 높은 경우 무리한 수술보다는 수술 전 항암요법이나 방사선요법 후 영상학적으로 진행하지 않거나 개선되어 혈관침습이 국소적인 경우 혈관 절제나 재건을 포함한 근치적 수술을 시도하여 비교적 좋은 결과들이 보고되고 있다. 하지만 아직 그 정의에서 각 병원마다 차이가 많고 치료 및 효과의 판정에서도 확립되지 않은 부분이 많아 잘 짜여진 기준을 가진 전향적 무작위 대조 연구가 필요하다(Table 5)[61]. 또한 수술 전 항암 또는 방사선치료효과를 정확히 평가할 수 있는 영상검사나 병리학적 기준이 필요하며 향후 개개인에 맞는 맞춤형 치료의 개발 및 효과를 볼 수 있는 환자군을 선택하는 것이 필요하겠다. 따라서 경험 많은 소화기내시경의사, 외과, 방사선종양학과, 영상의학과, 병리과 의사들의 최적화된 다학적 팀으로서의 접근이 필요하다.
